Use the below form to request a declarations page from your policy and we’ll get to work on it right away. You can also contact us between the hours of 8:30am and 5pm, Monday – Friday.
What is a Declarations Page?
A dec page specifies the most relevant info about your insurance policy. It includes the named insured, address, coverage limits, policy period, and more. It is used to prove insurance coverage and is often required by lenders.
Example: An auto loan typically requires that the bank is listed on the policy as a lienholder. Physical damage coverage is also usually required, to ensure the bank is paid out if the vehicle is damaged or totaled. A dec page will provide this info to the lender.
